Messages from the SHA Leadership Team

It is important for all of us to recognize the significance and meaning of the National Day for Truth and Reconciliation and Orange Shirt Day on September 30. On this day we honour residential school Survivors, remember the children who never returned home, and reflect on our role in advancing reconciliation.

On September 30, please:

1. Include a moment of learning or reflection as part of your huddles/team meetings.
2. Prioritize attending SHA-led or community events dedicated to Truth and Reconciliation.

While September 30 is important, we also acknowledge that reconciliation requires daily focus. In lead up to September 30, please initiate conversations with our teams about how to demonstrate reconciliation through our daily actions, relationships, and commitment to culturally responsive care. SHA leaders are encouraged to use discussion questions such as:

  • What does reconciliation mean in the context of our work and the people we serve?
  • How do our daily actions contribute to a culturally safe and respectful environment for First Nations and Métis patients, families, communities, and colleagues?
  • What have you learned over the past year that has changed or strengthened your understanding of Truth and Reconciliation?
  • What is one action, practice, or commitment you can take forward to support reconciliation in your role?
  • How can our team continue to turn learning and reflection into meaningful action throughout the year?

Consider closing each discussion by inviting each team member to share one takeaway or one commitment they will carry forward in their work. Encourage each team member to take the It Starts with Me pledge to advance their commitment to Truth and Reconciliation online or through a downloadable pledge form. This aligns with our shared goal of turning our commitment into action through reflection, learning, and everyday practices.

 Clinical Documentation

Provincial Nursing Foundational Forms now available

Clinical Standards has begun a phased rollout of standardized provincial clinical forms across acute care, although these forms can be used across all care settings.  This project will lead to more consistent and streamlined documentation across sites.

The initiative aims to reduce duplication, simplify workflows through fewer standardized forms, improve quality and patient safety, support audit and legal requirements, and prepare for future electronic charting.

Continuing with this phased roll out, the following clinical forms are rolling out September 29, 2026:

  • Discharge Care Plan (SHA 0798)
  • Nursing Care Plan - 2 pages (SHA 0790)
  • Nursing Care Plan - 4 pages (SHA 0791)
  • Nursing Care Plan - 6 pages (SHA 0792)
  • Nursing Care Plan - Supplement (SHA 0793)
  • Patient Belongings Form (SHA 0794)
  • Patient Belongings Indicator Card (SHA 0799)
  • Practitioner Ask List - Practitioner Version (SHA 0795)
  • Practitioner Ask List - Patient Chart Version (SHA 0796)
  • Practitioner Ask List - Unit/Charge Nurse Version (SHA 0797)

Staff are asked to transition to the new forms once current stock is used. This approach reduces waste, minimizes workflow disruption, and supports a smooth transition.

The following huddle talks are available to support the use of these new forms.

  • Discharge Care Plan SHA 0798 - Huddle Talk (CS-G-0546)
  • Nursing Care Plan - Huddle Talk (CS-G-0551)
  • Practitioner Ask Lists - Huddle Talk (CS-G-0548)

All provincial clinical forms can be found in the SHA Clinical Forms Catalogue.  An informational bulletin has been developed to support clinical team communication. Forms have been sent to Forms on Demand, SHA Printing Services and DCM Printing and will be ready for order/printing as soon as they are uploaded into these systems.

Customization/prepopulating the nursing care plans and discharge care plan are possible. Follow the guidance document below for more information and guidance.
