Alums Consult Service

The Alums Consult Service helps connect Alum with current Cohort physicians, SQI project leads, and fellow Alums to share their perspectives, insights and experiences. It supports our "Community of Experts"  to keep growing and becoming a place where people can collaborate on tricky QI issues. 

Each Cohort project has 15 hours of sessional funding available for Alum time. Consider connecting with one or more of the Cohort 9 physicians!

Well Planned Action Series

Thank you to everyone who participated in the Well Planned Summit event on June 26! We are working on pulling together a report to share with attendees and the larger Alum community. The results from the Summit will form the basis of our next Action Series, which will focus on medical goals for discharge.

Highlights:

  • An Action Series is a learning opportunity for small groups of interdisciplinary team members. It consists of five theme-based video modules and activity guides that can be completed asynchronously or together
     
  • Eligibility criteria: clinically active hospital-based Medical Staff (physicians/ NPs), Nurses (RN/LPN), and Allied Health (SW/PT/OT/SLP, Liaison, etc.); Patient Partners welcomed
     
  • Medical goals for discharge describe the clinical status of the patient that the team is working towards in order for the patient to be medically fit for discharge and provides information to the entire team of the clinical reason for the admission. Medical goals for discharge is not a checklist to be achieved before discharge (criteria led discharge), or a component of advanced care planning (goals of care)
     
  • This is an opportunity for PQI Alums to lead and contribute to system-level change. Recruitment to participate in the Action Series with your team launches the week of September 16
