We are excited to share that recruitment to participate in the Well Planned Action Series has officially launched. 


The Well Planned Action Series will provide support to improve the experience for people Creating, Clarifying, and Communicating Medical Goals for Discharge. It is a flexible, low time commitment opportunity to learn some tools and test your high impact-low effort ideas for improvement with a small multidisciplinary team. Each area has unique challenges and the people who are doing the work are the best situated to know what change ideas make sense to try in their area.

Action Series Highlights:

  • 5 Modules spread over 5 months (October 20254 – February 2026); approx. 10-20 mins of videos and 45 mins of activities in each module, which can be done in as little as 5-10 minute segments
  • Participate in multidisciplinary teams of 3-7 people who are involved in creating, communicating, or clarifying medical goals for discharge 
    • Must include at least one physician and one non-physician role, including Nurse Practitioners, Midwives, Nurses, Allied Health professionals, etc.
    • Example team compositions: A) 3 RNs, 2 CNLs & 2 Physicians;  B) 2 NPs, 1 Physician & 2 LPNs;  C) 1 Physician, 1 CNE & 1 SLP

If you are interested in participating but aren’t sure about building a team, we recommend asking two friends/colleagues you work with to try it with you. There is no “team lead” role and no additional responsibilities for the person who pulls a team together. Reach out to PQI@islandhealth.ca if you need help identifying a team.

 

Please note: Medical Goals for Discharge provide information about the clinical reason for a patient’s admission and describe the clinical status the patient and team are working towards in order to be medically fit for discharge. Medical goals for discharge is not a checklist to be achieved before discharge (criteria led discharge), or a component of advanced care planning (goals of care).
