Loss of control and collision with terrain: TSB report A22C0027

The Transportation Safety Board of Canada (TSB) has released its investigation report (A22C0027) into the fatal collision with terrain involving a privately registered Piper Cherokee PA-28-140 aircraft near Sioux Lookout in 2022.

At 2102 on 29 April 2022, the privately registered Piper Cherokee PA-28-140 departed Dryden Regional Airport (CYHD), Ontario, bound for Marathon Aerodrome (CYSP), Ontario, with 1 pilot and 3 passengers on board. The aircraft was reported overdue at 0139 on 30 April 2022. The aircraft had last been observed on radar at 2145, flying a zig-zag route over an area with limited cultural lighting as shown above. A search was initiated and the wreckage was found approximately 19 nautical miles south-southeast of Sioux Lookout Airport (CYXL), Ontario. The aircraft had impacted terrain in a wooded area during the hours of darkness. The pilot and 3 passengers were fatally injured. The aircraft was destroyed. The emergency locator transmitter (ELT) activated on impact and the signal assisted search and rescue services in locating the accident site.

The conclusion of this report:

Continued flight under night VFR into areas with reduced visual cues, such as areas with limited cultural lighting or deteriorating weather, can lead to spatial disorientation and a loss of control. All pilots—no matter how experienced they are—need to plan ahead and consider strategies to avoid such conditions, as well as have alternate plans should such conditions be encountered.

MF frequency at Saint-Hyacinthe airport (CSU3)

Some of our readers may be surprised to learn that since June 17, 2021, the frequency classification in the 5 NM zone around Saint-Hyacinthe airport is now an MF frequency. 

The frequency remains 123.2 MHz, 5 NM 3200 ASL, but is now classified as MF.

The use of an MF frequency is governed by the CARs under sections  602.97 to 602.104.  For your convenience, we reproduce here the text of CAR 602.97:

602.97 (1) Subject to subsection (3), no pilot-in-command shall operate a VFR or IFR aircraft within an MF area unless the aircraft is equipped with radiocommunication equipment pursuant to Subpart 5.

  • (2) The pilot-in-command of a VFR or IFR aircraft operating within an MF area shall maintain a listening watch on the mandatory frequency specified for use in the MF area.

  • (3) The pilot-in-command of a VFR aircraft that is not equipped with the radiocommunication equipment referred to in subsection (1) may operate the aircraft to or from an uncontrolled aerodrome that lies within an MF area if

    • (a) a ground station is in operation at the aerodrome;

    • (b) prior notice of the pilot-in-command’s intention to operate the aircraft at the aerodrome has been given to the ground station;

    • (c) when conducting a take-off, the pilot-in-command ascertains by visual observation that there is no likelihood of collision with another aircraft or a vehicle during take-off; and

    • (d) when approaching for a landing, the aircraft enters the aerodrome traffic circuit from a position that will require it to complete two sides of a rectangular circuit before turning onto the final approach path.

A resounding success: the dinner-conference for the benefit of the Fondation Aérovision Québec with Captain Robert Piché

On November 17, a superb dinner-conference by Commander Robert Piché at St-Hubert airport as part of an evening benefiting the Fondation Aérovision Québec . Captain Piché was able to demonstrate his talents as a narrator and captivate the guests with the true story of Air Transat flight 236 on August 24, 2001. 

This event raised more than $20,000 for the benefit of the Foundation. Aviateurs.Québec was also well represented with 4 directors present. Jonathan Beauchesne accompanied by Kitrie Marin-Auger wore the colors of COPA.

Do you know the Quebec Aerospace Museum?

A unique project in Quebec 

The Quebec Aerospace Museum is a non-profit organization that was created in August 2018 with the mission of preserving and promoting aerospace heritage in all its forms.

It is in this capacity that the NPO wishes to be the project manager for an aeronautics and space museum project to be set up at Saint-Hubert airport. To date, apart from the Montreal Aviation Museum in Sainte-Anne-de-Bellevue, there is no major museum in the Montreal metropolitan area.

An original vision

Achieving the MAQ's mission involves several objectives, including positioning the aerospace industry at the center of the Museum's evolution and integrating educational and community components while ensuring the promotion of aeronautics and space for young people and the general public.

A direct return to the community makes the Quebec Aerospace Museum an organization with added value from a social, academic and economic point of view in the aeronautical sphere of Quebec.

Moreover, among the mandates that the MAQ has set itself, there is also the promotion of the place of women in the aerospace industry in Quebec and Canada as well as the promotion of aeronautics within the First Nations and Inuit.
