A space is available in a three-week quality learning series that is designed for those who wish to deepen their knowledge of the core components of quality as defined in the BC Health Quality Matrix and increase their confidence in applying the framework within their engagements.

Member, Palliative Care Advisory Group
As a member of the advisory group, you will have an opportunity to describe your experience with palliative care in the Vernon, Armstrong, Enderby, Lumby or surrounding area and to describe and evaluate the existing palliative care services, as well as identify opportunities for improvement. The goal of the group is to develop a new model of care and future processes that support effective communication and transitions between care settings (home, long term care, hospital, hospice).

Goals of Care
Regional Palliative Approach team is working to improve transitions between community and acute care (for clients being followed by home health, and is then readmitted to the hospital and discharged back home). The team is working to develop standardized workflows and guidelines that improve the flow of documented information around health status and health care conversations between community and acute care.

Review of Regional Home Support Pamphlet & Client Handbook
The Home Support (HS) pamphlet is designed for anyone who needs to know more about Home Support and what kind of services it provides. The current HS pamphlet has not been updated since 2011, and there is no regional HS client handbook, so the team is developing materials that will be provided to new home support clients so they have all the most important and relevant information they need to be informed about the services they are receiving.
