Lab Testing

Wondering how to test your kidney function?

The most common test for kidney function is the calculated test: glomerular filtration rate (eGFR).  It is based on the amount of the waste protein CREATININE in your blood. (Not to be confused with the supplement amino creatine.)  More waste (high creatinine) in your blood shows that your kidney didn't do a good job of filtering it out and passing it to your urine - resulting in lower filtration results (lower eGFR).

Now, for the most part anything above a 60 for your eGFR is pretty great, BUT it's even more wonderful to hit 90+.  Once you dip below 60, we begin to keep a closer eye on kidney function.  A second test that can be looked at early is a urine microalbumin test - also known as the albumin:creatinine ratio or ACR.  Did you know?  A first morning specimen is preferred.  This test might pick up slight changes in kidney function that weren't as obvious on the eGFR.  For a better idea of how these 2 values (eGFR and ACR) can be mapped into an overview of function see:

The Case for Hyperthermia in Cancer Treatment

Meta-analyses are generally considered the highest level of evidence in research, combining multiple studies on the same subject to make more robust conclusions on what the findings are. Hyperthermia has undergone many meta-analyses over the years, and I will highlight two of them here.

 

Yea JW, Park JW, Oh SA, Park J. Chemoradiotherapy with hyperthermia versus chemoradiotherapy alone in locally advanced cervical cancer: a systematic review and meta-analysis. Int J Hyperthermia. 2021;38(1):1333-1340.

  • This meta-analysis looked at the synergistic effect of hyperthermia (HT) with standard chemoradiotherapy (CCRT) on locally advanced cervical cancer (LACC) patients.
  • The CCRT + HT group had significantly better five-year OS than the CCRT group (HR 0.67, 95% confidence interval [CI] 0.47-0.96, p = 0.03).
  • This systematic review and meta-analysis showed that CCRT + HT significantly improved overall survival in LACC patients without increasing acute and chronic toxicity

 

Datta NR, Rogers S, Ordóñez SG, Puric E, Bodis S. Hyperthermia and radiotherapy in the management of head and neck cancers: A systematic review and meta-analysis. Int J Hyperthermia. 2016;32(1):31-40.

  • A systematic review and meta-analysis was conducted to evaluate the outcome of controlled clinical trials in head and neck cancers (HNCs) using hyperthermia (HT) and radiotherapy (RT) versus RT alone.
  • In total 451 clinical cases from six studies were included in the meta-analysis. The overall complete response (CR) with RT alone was 39.6% (92/232).
  • With HT + RT the overall CR was 62.5% (137/219), (range 33.9-83.3%). The odds ratio was 2.92 (95% CI: 1.58-5.42, p = 0.001); the risk ratio was 1.61 (95% CI: 1.32-1.97, p < 0.0001) and the risk difference was 0.25 (95% CI: 0.12-0.39, p < 0.0001).
  • Acute and late grade III/IV toxicities were reported to be similar in both the groups.
  • HT + RT enhances the likelihood of CR in HNCs by around 25% compared to RT alone with no significant additional acute and late morbidities. This level I evidence should justify the integration of hyperthermia into the multimodality therapy of HNCs.

 

These are two examples of the level of evidence in support of adding hyperthermia to the standard of care. Adding hyperthermia to chemotherapy, radiotherapy, and other standard treatments is both safe and effective, across dozens of cancer types.

Doctor Favourite Receipes

Greek Meatballs and Tzatziki

Meatballs

  • 1 lb ground turkey or chicken
  • 1 egg
  • ½ cup breadcrumbs
  • ¼ finely chopped red onions
  • 3 cloves of minced garlic
  • 3 tbsp fresh parsley (or 3 tsp dried)
  • 1 tbsp fresh mint (or 1 tsp)
  • ½ tsp dried oregano
  • ¾ teaspoon sea salt

Veggies

  • 2 bell peppers (red or orange or yellow), julienned
  • 1 red onion, sliced
  • 1 tbsp extra virgin olive oil
  • Salt and pepper to tase.

Tzatziki

  • ½ cup finely grated cucumber
  • 1 cup plain Greek yogurt
  • 1 tbsp fresh lemon juice
  • ½ tbsp extra virgin olive oil
  • 1 garlic clove, grated
  • ¼ tsp sea salt
  • 1 tbsp chopped fresh dill (or 1 tsp dried)
  • 1 tbsp chopped fresh mint, optional

Instructions:

  1. Preheat the oven to 400 degrees Fahrenheit and line a baking sheet with parchment paper.
  2. Combine the ground meat, egg, breadcrumbs, red onion, garlic, herbs, salt, and pepper. Using clean hands, form approximately 16 meatballs, each about the size of a golf ball. Place them onto the prepared pan.
  3. Add the peppers and onions to a bowl, then drizzle with olive oil, salt, and pepper. Toss the vegetables to coat them evenly and transfer them to the prepared pan spreading evenly around the meatballs.
  4. Bake for 17-20 minutes or until the meat thermometer reads 165 degrees Fahrenheit.
  5. While the meatballs are cooking, prepare the tzatziki.
  6. Using a paper towel, squeeze out the excess water from the grated cucumber.
  7. Combine the cucumber, yogurt, lemon juice, olive oil, garlic, dill, and mint. Once thoroughly mixed, chill until ready to use.
  8. Once the meatballs and veggies are cooked, serve them with rice, quinoa, or pita bread, and top with tzatziki.
